I do layouts in Dreamweaver, and I start without any images. I have images I want to use in mind, but I wait to place them until the layout has started to come together. I usually place background images first and work towards the front. Sort of setting the foundation before I put the finishing touches on with the header images, etc. Does that answer your question?
